You can either fill this form out online then print it out, or print it out first and fill it out by hand. Please include one order form in each batch of fiber. A "batch" is fiber that will be processed the same way as one order. So, for example, if you had 3 alpaca fleeces to be processed into roving and 5 wool fleeces to be processed into yarn, you would have two batches, and hence two order forms. Questions?
